Diabetes Education Program
Diabetes is a disease marked by high blood sugar levels as a result of either the pancreas not making enough insulin or cells not responding to the insulin that is produced.
At Horn Memorial Hospital we have a team dedicated to helping you understand and manage your diabetes.
Your diabetes care team includes Registered Nurse Diabetes Educators and a Registered Dietician who has been approved by the Association of Diabetes Care and Education Specialists (ADCES) to work together with you and your provider to:
Provide education about diabetes
Assist you in setting personalized goals to improve your health and minimize your risk of diabetes-related complications
Help you develop a plan for meeting your goals
Work with and support you through face-to-face visits, telehealth visits and follow-up phone calls
Stay connected to others with diabetes by joining a free online diabetes support group
Connect you with other diabetic resources, such as a Pharmacist or a Registered Dietician. The Pharmacist can assist you with understanding your medications by learning the purpose, use and side effects. The registered dietician at Horn Memorial Hospital can teach you the skills that will help you manage your diabetes by discussing how your food choices and diet affect both your blood sugar and the risk of diabetes-related complications.
All of this will help you become an "expert" in the management of your diabetes!
